Direct Implant Risk Factors

The factors below are directly associated with a higher rate of implant failure within 5 years. Risk multipliers shown are based on published studies. Patients with more than one risk factor may face compounded risk.

Diabetes

Patients with poorly controlled diabetes have a 2.7× greater risk of dental implant failure. Elevated blood sugar levels can slow healing, reduce the body's ability to fight infection, and interfere with the bone's ability to successfully integrate with the implant. Maintaining good glycemic control before and after implant surgery can significantly improve treatment outcomes.

Head and Neck Radiation

Prior radiation therapy to the head and neck carries a 2.7× greater risk of failure, as radiation damages bone vascularity and the body's ability to integrate the implant.

Corticosteroids

Prednisone, Prednisolone, and Dexamethasone are used for rheumatoid arthritis, asthma, and inflammatory bowel disease. They reduce the body's ability to absorb calcium and accelerate bone resorption — both harmful to implant integration.

Smoking

Contributes to poor metabolism, impaired healing, and reduced blood flow to bone. Also associated with earlier onset of menopause in women, increasing bone loss risk sooner.

High Alcohol Intake

Limits the stomach's ability to produce hydrochloric acid needed for calcium and magnesium absorption. When magnesium is lost in urine, it takes calcium with it — weakening the bone environment around the implant.

General Bone Loss Risk Factors

The factors below are associated with general bone density loss (osteoporosis). While they don’t directly cause implant failure, they may combine with the direct risk factors above to further increase overall risk. Do not stop any medication without consulting your physician.

Anticoagulants

Warfarin (Coumadin), Heparin

Reduce the effects of vitamin K, which helps calcium bind to bone tissue. Used for heart disease, blood clot prevention, and post-surgical recovery.

Anticonvulsants

Carbamazepine (Tegretol), Pregabalin (Lyrica)

Interfere with vitamin D metabolism, reducing the body’s ability to absorb calcium. Used for epilepsy, bipolar disorder, fibromyalgia, and nerve pain.

Anti-Anxiety Medications

Clonazepam (Klonopin), Diazepam (Valium), Lorazepam (Ativan)

Inhibit vitamin D metabolism, leading to reduced calcium absorption and gradual bone loss.

Aromatase Inhibitors

Used in breast and ovarian cancer treatment

Disrupt estrogen production as part of cancer treatment, leading to accelerated bone loss as a side effect in post-menopausal women.

Diuretics

Furosemide (Lasix)

Cause the kidneys to excrete more calcium, contributing to bone loss over time. Prescribed for high blood pressure, heart failure, and kidney or liver disease.

Immunosuppressants

Used after organ transplant; also for psoriasis, Crohn's disease

Hinder the formation of new bone while suppressing immune reactions to prevent rejection or treat autoimmune conditions.

Kidney Dialysis Solutions & Antacids

Prilosec, Prevacid, Nexium

Contain aluminum or reduce stomach acid, disrupting the calcium-phosphorus balance needed for bone formation and interfering with calcium absorption.

Oral Contraceptives

Birth control pills

May elevate sex hormone binding globulin (SHBG) levels — an effect that can persist after stopping the pill — potentially impairing androgen activity and contributing to osteoporosis.

Lack of Exercise

Sedentary lifestyle

Lack of weight-bearing exercise accelerates bone loss. “Use it or lose it” applies directly to bone health — regular physical activity is one of the most effective tools for preserving bone density.
